I could tell my wife but can't

My wife knows i am bi after telling her and she even let me have sex with a man she has also met. Now you don't have to say it i am fucked up and an idiot. I met another guy who i found out is in a wheelchair. I had sex with him because i felt sorry for him and this morning early unprotected sex. I can't tell my wife because his caretaker is his sister who goes into the next room when me and him have sex. I am not attracted to him but i feel he needed it. This morning he got behind me and i put the condom aside. His sister can hear us but when he got close i whispered go ahead. I am not sure how i feel right now and want to tell my wife but even though there is no sex with his sister i think i will ruin my marriage. They have no family and friends and don't want to stop seeing him.
